By checking the box below, I acknowledge that baseball and athletic activities involve risk of injury, including injury from balls, bats, equipment, running, throwing, hitting, fielding, and other participants.
I voluntarily allow my child to participate in Hit Factory Los Angeles camps and assume these risks. I release and hold harmless Hit Factory Los Angeles, its owners, coaches, employees, and representatives from claims arising from ordinary negligence, to the fullest extent permitted by law.
I authorize emergency medical care if reasonably necessary.
I also give Hit Factory Los Angeles permission to photograph and/or record my child during camp and use those images or videos for website, social media, advertising, and promotional purposes without compensation.
